New Patients

Please bring your current insurance card and an identification card, such as a driver license. Patient registration will be done on an iPad that will be provided for you.

Return Patients

We have implemented a new medical record system since February of 2020. If you have filled out your information on the iPad, you are all set to go. If not, we may need you to fill out some information once.
